will not take medicine

My spouse received her diagnosis about 2 and half years ago. she is not taking her prescribed medicine and if i raise the concern it is instant flare up and defensiveness. Her dementia of the altzheimer type is complicated with depression as a result of child trauma. My quesiton is how to approach the subject of taking medication without getting into an emotional battleground.

    also we have tried home health to assist in med taking and she told the home health person that she was not needed.
