Doing it alone Vascular dementia

My mom has vascular dementia and like many does not think she has any memory issues . She refused to go to the doctor and lives alone in a huge house with her dogs . I have someone who comes 2x a week but is usually asked to leave early as my mother does not like company. The biggest hurdle right now is the one dog who bites people . It is nearly impossible to get a caregiver because of the dog . I know it would be better to rehome him but at what cost to my mother as he is like family. Any thoughts?
