My brother has vascular dementia and seems to be declining rapidly

My brother and I got close after our parents died. We basically only have each other . No other siblings or close relatives . He had some minor memory problems starting about 1 1/2 years ago but 6 months ago he seemed to get more forgetful. He was diagnosed with vascular dementia only 3 months ago and has rapidly been getting worse . We live in different cities . I have been taking over more and more. He has 2 caregivers 24 hours a day. He uses a walker and is a high fall risk. He resists getting his disposable underwear changed, resists taking medication often saying it make him worse. Everything overwhelms him . He says no one should pressure him. He insists he is being threatened and his cell phone has 2 systems and can’t work. He is frightened but I don’t know how to help him. Frankly, I feel bad saying that I often find myself crying because I miss him and all of this is new for me. I feel very alone. We used to turn to each other . I have friends but no one seems to understand the pain and fear I am going through . And I feel guilty when I focus on my pain and not his. I don’t know what to expect next but I know this is a very difficult path
