Knee surgery for PWD?
Both of my DH's knees are severely deteriorated and he qualifies for knee replacement surgery. He has MCI but most likely has progressed to stage 4. I'm terrified to have him go through the surgery because 1) I've read that general anesthesia can cause further cognitive decline, and 2) I'm not sure that he will have the ability to do the critical physical therapy required after surgery. So far, he had successfully gone through a few rounds of cortisone shots, which seems to help for a short time, but there's a limit to how often they will give the injections. Now he's starting to talk again about getting the surgery, at least in one knee to start. He is in tremendous pain most of the time, but I hate the thought that he may actually be in worse shape, physically and intellectually, after the surgery. I suspect that some folks in this group have been through similar circumstances; any advice?

Ask the physician if procedure can be done under spinal anesthesia.

My husband should have had knee replacement surgery well before he was diagnosed with Alzheimer's but he kept saying they were fine (they weren't). So he didn't. After the diagnosis, and as his walking deteriorated , we tried the cortisone shots but his arthritis was so far gone they did no good. I knew how hard the recovery from the surgery was, having watched my cousin the weeks after the surgery and felt that getting him to do the p.t. would be impossible, plus the effect of the anesthesia on the dementia would be bad (I read all the literature on this; it clearly is bad). So now he can barely walk a few blocks, slow and shuffling, though I make him walk every day to keep what mobility he has. It makes life even harder, the dementia and the physical impairment. Looking back, maybe I should have pushed for the surgery when he was in the MCI stage, but that's water under the bridge. He's now more or less stage 6, needs help with all ADLs, teeters on the three steps in our house, and walks about snail speed. But it's also hard to separate the effects of Alzheimer's on his gait (not knowing where his body is in space—not perceiving that the carpet and the floor really are just one continuous suface, not being able to take bigger steps) from the really bad knees. There is a procedure we looked at which supposedly gives pain relief for a year or two—genicular artery embolization—but I decided that the actual procedure too stressful so we didn't do it. But look into it if you are in an urban area. Good luck!

My DH is in moderate stage and he is in week 6 post after surgery for knee replacement. I regret that we did not do it a year ago. It got to the point where he could not walk short distances. So that kept us from getting him out more which I think helps everyone to get fresh air and out of the house. We tried everything including gel injections and nothing helped. He could not wait to have surgery.
The doctor said most patients when the pain gets so bad they are wanting to get it done sooner.
Our surgeon did a spinal block to avoid anesthesia and that was good. The first week is the hardest, but he and you will see how it improves with Pt. He goes 3 times a week. I told him it is the doctor’s orders. We have a exercise equipment so I am hoping he will use it when ready. He has less pain, but understand it may take months for to recover. Everyone is different with recovery. Our therapists are friendly and he enjoys that part of it.
I know it was a big decision for me. His memory is bad anyway, so did not see much decline. Getting him weaned off the medications helped. He is limited to 4 Tylenol a day because of liver issues.
