Yes, most states have this, but the person has to have Medicaid in place. Not Medicare, as your title says. It's difficult to qualify for Medicaid while in the community, income and asset levels are low, it's different than long term care Medicaid. But if the person with dementia does have it, then a family member can get some payment.

"Interested parties should be aware that in all but fifteen states, Medicaid prohibits legal guardians and spouses from being paid. Notably, Medicaid does not prohibit the adult children from being hired and compensated.
