    So in one year only 15 providers responded to the survey. You can't change a system with that lack of interest. The approach needs to change.

     What I think is most important about that document is the codes in the back.

    I agree. It would be wonderful if everyone copied and got to their Dr.

DH = Dear Husband
DW= Dear Wife, Darling Wife
LO = Loved One
ES = Early Stage
EO = Early Onset
FTD = Frontotemporal Dementia
VD = Vascular Dementia
MC = Memory Care
AL = Assisted Living
POA = Power of Attorney
