Home care for incontinent spouse with bed sores.

My DH developed nearly complete urinary incontinence this summer.  Before then, he'd have accidents, but they were infrequent.  He has mobility issues, so I bought him urinals to have beside his chair.  And I check with him every couple of hours to see if he needs to go.  I don't think that he can tell if he needs to urinate, and if he goes in the diaper, he doesn't know that he is wet.  As a result, he developed rashes and sores on his bottom.   From this experience, I learned that he can't tell me if his diaper is wet, so I have to check every couple of hours and help him shift positions.  For now the goal is to keep him clean and dry.  I use diaper rash cream with zinc to add protection.  Is there anything else that I can try?

My grandmother use to have the same issues. She is fully incompetent. I bought a mattress for her bed. It’s like an air mattress that moves air every 6 minutes. We’ve been using it for three months. Haven’t had another bed sore.
