Backup Care Plan Resources

In a recent post, I mentioned a backup care plan that I was working on with the help of a support program that we are enrolled in.  While I don’t feel that it is appropriate to share their copyrighted document, I thought I could do a search for similar resources online and post this for the group. 

My Google Search Term was:  caregiver backup plan

Here are some of the more promising sites.  Some have forms; others just list topics to be included. Creation of a notebook is mentioned – and it what my organization recommends.  I’m also keeping the documents on my computer so that I can update and reprint as updates are needed.  You’ll find that some of the topics do not apply to your situation at present and that some of the financial information may not be appropriate for a substitute in-home caregiver but may be needed for the person with the financial POA – so dividing the information into different forms may be needed.

One thing that I found missing on most forms is a place to write down the location of the prescription medicine bottles – important, if like me, you count out the meds in a daily pill dispenser and keep the remainder of the bottles in another location.

- Not a backup plan, but has information that would be part of what would be documented in the daily routine section of the plan. https://www.alz.org/help-support/caregiving/daily-care/daily-care-plan

- Not a backup plan – but the document has a form at the end for thinking about information that is needed.  https://www.fidelity.com/bin-public/060_www_fidelity_com/documents/Aging_Well_Guide.pdf
