    Have the doctor’s ruled out a nerve issue such as neuropathy?  How has your wife described her foot pain?  My mom was finally diagnosed with neuropathy by a neuropathy specialist.   Her symptoms:  numbness, burning, tingling.  She complained about them hurting when she walked, so she reduced the distance she walked.  She walks to get where she needs to go, but she no longer walks just to walk or get exercise.  She complained about the various issues of shoes she has hurting her feet,  buying her new shoes didn’t help. 

    Her family doctor finally put her on 100mg of gabapentin 3 times a day due to the length of time it took to get in to a neuropathy specialist at the hospital here. That didn’t help.  The neuropathy specialist appointment  finally came and he diagnosed her, grimaced ( laughed?) at that dosage and immediately increased it to 300mg 3 times a day.    

    FYI-  the specialist and his resident  had her do a quick test to finalize the diagnosis. They stood  a few inches from her, arms around her so they could quickly grab her.  They had her stand, prove she could stand,  and then had close her eyes. She immediately started  leaning( falling?)  to one side.  Basically this tells them her brain can’t figure out her body position without  her eyes..   The rest of us ( without physical limitations of course) can continue to  stand even with our eyes closed. There is a name for this test, which I’ve forgotten at the moment).

    Has she ever had an ankle or foot injury when she was younger?  Broken bone, tendon strain/tear, ligament damage?  Was she ever athletic, played a sport?

    I ask because even though it's been over 20 years, I broke my 5th metatarsal back then and especially when it's cold it really hurts.  I don't limp or anything, it's just an ache that at times can be described as painful.  Winter is tough with this.

    Given the weight gain, I wonder if she hasn't some arthritis that has worsened with age and extra weight.

    I have had plantar fasciitis in both feet for decades, treated with orthotics and supportive shoes.  Just in the past six months or so, I have had sharp pains in my right foot, in the heel and in the metatarsal area.   I have no swelling or tenderness.  I have gained a lot of weight in the past year.  I believe my (R) foot pain is due to plantar fasciitis+being overweight.  Plantar fasciitis can cause persistent pain.  A visit to a podiatrist may help.

    About 2 weeks ago, I was awakened around 3 A.M. with severe pain in my right ankle. It literally felt like somebody was driving a 16 penny nail in my ankle. The pains were shooting, not constant. I got out of bed, and the pains came 4 or 5 more times, then quit. I went back to bed, and went to sleep. The next night I had the same thing, only not as intense. Again they stopped. Off to ER because I thought it might be a blood clot. Doc quickly dismissed that, but ran tests for stress fractures, arthritis, and for gout. Nothing showed up. Symptoms lasted off and on for 3 days, then just stopped completely. I have an appointment with a podiatrist next week. I honestly don't remember having pain that intense before.
