I'm so sorry you're going through this. Seeing our loved ones struggle is heartbreaking for sure. The trigger for my dad moving out of his apartment was not being able to drive or cook. He also liked going on walks, but would likely not be able to find his way back. He was able to transition to assisted living (then transferring to memory care) because he was able to walk, feed himself and handle his personal care. In memory care he's no longer able to do those things. Nursing homes are typically after a hospital stay when they require 24-hour medical care.
