Moving advice?!?

Hi family, i live with Mom (late stage Alzheimer's) and Dad. Dad is the main caregiver and we tag team her 24/7 care. We have an amazing aide who helps 4 days a week too. We are thinking of moving into a smaller home on one level in the same neighborhood a mile away. Our house now is way too big and two stories. Mom doesn't recognize our home most of the time or the stuff she had collected for years. To my Dad and I this makes great sense to move toward a forever home for the future. We plan to keep Mom at home as long as possible. Has anyone got experience, input or ideas for a smooth transition
