Beginning stages of incontinence

Hello, new to this forum. Looking for some advice. I am currently a caregiver to a sweet 93 year old woman with dementia.
She has had “accidents” here and there, but seems to be getting worse lately. I find messes in the bathroom where it looks like she’s tried to clean up. Also while doing laundry I find underwear with solids on them, which I toss. I do not want to embarrass her. She is still clinging on to any independence she can. But, it’s coming to a point where it a a sanitary issue. How do I start this conversation? Thank you.

Side note: I have cared for others in the past with incontinence, but they were either in later stages or willing to let me change them.
