Defiant Sister with Damentia Symptoms

I am a two year younger sister living 45min from my older sister who shows severe short term memory loss, reasoning abilities, comprehension difficulties and some inability to think of words. She defiantly refuses to go to a doctor because she thinks nothing is wrong with her. I am soooo stressed over not being able to help her. She only has one friend who sees her every day and he is getting very tired not being able to help her either. Are our hands tied until something drastic happens to her? She doesn’t wonder, and only drives to his house and back apron 3 miles. I am so so so frustrated what to do.
