You would be acting as a friend. He would need to designate someone (you) to be his POA for medical and for financial needs. You might need guardianship if he could not sign for POA. You would then attend doctor appointments and make decisions for him. You could admit him to residential care. You would admit him into hospice care and make his final arrangements. As M1 says, you need legal advice to accomplish what you want to do.
If you decide not to become his caregiver, and there is no family member or other friend willing to take on this challenge, he would have a court-appointed public guardian to manage his affairs. There are other members here who are friends of the PWD who become the caregiver. There are and have been also a few members caring for ex-spouses. They post on the Spouse/ Partner board. You might repost on that board and you might get more responses.
