Information in case of emergency

DH has ALZ. I read somewhere about posting info on 'fridge in case of emergency for emergency personnel to find. How much info is enough and how much is too much?

Emergency contacts, doctors, diagnosises, allergies, insurance, doctors… is this too much? Not enough?.

    Read about File of Life. It is to file your personal and medical information to leave on your refrigerator, where first responders know to look. I got mine for free at my local Area Agency on Aging.

    Also, check to see if Smart911 is available in your community.

    I created an account for my mom who doesn't have dementia but lives alone. A call to first responders— from her or her Apple watch— would pull up a whole file of information on her. I was able to include the number to her lockbox so they can open rather than break down the door. My contact information. Her PCP, medications, medical conditions (including the serial number, info on implanted materials (needed for MRI safety) and preferred hospital ER. They also ask about pets and allowed me to upload a picture of mom and the cat.

    Plus medical scope of treatment, including dnr if appropriate. I also have a copy of this in my glove box and a note that I'm a dementia caregiver for my husband.

    I have this on my phone's home page. Most of it is visible without having to unlock my phone.

    In Case Of Emergency:

    My name is Maria Xxxxxxxx.

    My husband is Jack X. Xxxxx

    If we are separated and I am not responsive, please take Jack to the ER at Txxxxx Hospital. His doctor is Axxx Hxxxxxxxx.

    Jack has dementia and has limited mobility.

    Our grandchild, who lives with us, is Kxxx (Xxxxxxx. 555 555 5555

    My sister Cxx Xxxxxx lives in Cxxxxxx. Her number is 555 555 5555

    She is my DPOA.

    You can also put medical orders like a POLST there. It's typically a pink sheet.
