Caregiver daily log with timestamps, for a caregiver agreement

Would anyone happen to have an example of a daily care log they keep? Tasks completed daily with times. I was told to keep records of everything I am doing and the times they were done. I am looking for an easy way to keep track of this. Any pictures of examples would be great.
