I have not seen my neurologist in a year and have declined frighteningly in last 12 months.

How do I explain all the changes?

Take someone with you that can help you explain what is happening

    Make a list 1 thru.... 10

Talk about the most concerning issue first.

Make a copy and give it to the neurologist and the nurse that helps.

Be honest! Don't sugar coat things

Ask for help. Medications, support.. etc.

Ask what stage are you in and what may happen next.

Plan. Plan plan... get your affairs in order!

    Wishing positive outcomes!

What has been suggested is spot on. One other thing I might add is to send the concerns to the neurologist through the patient portal several days prior to the appointment. That way they'll know your concerns.

I might add that if your noticing changes or something that concerns you between your yearly neurologist visit, make an appointment to seem them sooner. The neurologist works for you, to help you - if you don't share what's happening they can't help you.
