Having her room moved has been a theme with mom for a while (even in AL) but does anyone know why after I get her a shower, she routinely thinks we are now in a different room? It is like going in the shower changes her whole perspective.

Spatial disorientation is a very common symptom of dementia, and a PWD can feel lost even in very familiar places. Many people on this forum (including me) have had the experience of a PWD talking about “going home” even when they are in a place they’ve lived for decades. So as odd as it feels to have your mom imagining she is in a new room after being in the shower, it’s just her brain trying to make sense of her world.

Maybe she just feels different after showering and is interpreting the whole environment as having changed? I don’t know about that, but your story does match things I see with my dad.

Sadly it’s very common. Her memory is gone so she doesn’t recognize where she lives. You could try pointing to familiar things in her room like pictures, a throw, blanket, etc. that she’s had a long time.
