A facility will typically do their own assessment of the PWD before admission to determine the appropriate level of care. This is to make sure the facility is a good placement for the patient and the level of care required also determines the cost.

My husband's doctor had to fill out forms for the MC. In there he stated that my DH has dementia.

I'm in the process now. Our primary care NP is doing a physical and completing forms tomorrow. We are seeing the facility DON Thursday. Their assessments will determine his care needs.

Yes, you do need a diagnosis by a doctor, or at least verification of ADLs that the resident is incapable of doing any longer. Prior to accepting into any mcf, a form (aka doctor's report) must be filled out by a doctor, including negative test results for tuberculosis. The MCF will review and determine whether the patient is accepted as resident or a "right fit" for their community. Beware that many facilities won't accept patients with LBD or FTD. If accepted, a care plan will be developed accordingly.

the first facility I placed my DH in sent a nurse to our home to do the assessment. She took down the names of his doctors at the VA.

This is what I went through to get my DW admitted to her MCF:
- I talked to the admissions director to discuss whether the MCF was a good fit.
- I filled out a lot of paperwork.
- The admissions director and assistant director of nursing came to our house to do an assessment.
- After I was told DW was a good fit for the MCF, her PCP needed to examine her and then fill out paperwork. The MCF also needed to get DW's medical records. "Alzheimer's" was already in her medical records. DW also needed to get a chest X-ray to rule out TB. I had to do a little creative fibbing to explain why the exam and X-ray were necessary.
- After T's were crossed and I's dotted (and, oh yes, payment arrangements made), the MCF and I agreed on when to move DW to the facility.
