Advocating for Mom from far away

My mom is showing many signs of dementia, but does not have a formal diagnosis yet. I am trying to help take care of her from a distance, and make and manage her doctor appointments. There is no one to go to doctor appointments with her, and I am concerned that Mom is avoiding talking to her doctor about her memory issues, so her doctor does not know how much her memory has declined in the past couple of years.
Any advice on how to advocate for Mom from far away, to make sure the doctor is well informed and Mom gets the help she needs?
