Has anyone used Companioa, a program where the goal is to stay at home (as opposed to AL or MC), by using a combination of "care coaching" for caregivers, some home care, adult day care, and care management (medication, medical appointments, etc.). It seems to be a middle ground between 1) doing caregiving totally on your own and 2) using something like MC or AL.

This is offered by the Institute of Aging (IOA) in San Francisco, California but there may be similar programs elsewhere.
